Genealogy Search.

Office will respond to a request for a record without an exact
date, subject to the fee schedule of the NYS Dept. of Health.

You may request vital records in person or by mail.

Please include SASE.
Money must be sent with a request.
It is acceptable to use the New York State Genealogy Application form to request a record.
The town does not have an official form of its own.

The record sent to you is abstracted information filled in on a standard form.
If the records are not on file, a “no record” form will be sent.

It usually takes 10 days to answer a written request for a record.
