Your are asked to use an official form to request a vital record - the Town of Fishkill form is the same as the
New York State Genealogy Application form.

A vital record for genealogy purposes is usually sent to you as abstracted information filled in on a standard form; upon request an exact transcript of the original will be sent. For death records, a photocopy of the original entry is sent.

Requests are handled as they arrive; response may be slowed during busy times of the year (e.g., when hunting licenses are being sold).
