You may request vital records in person or by mail.

Please include SASE.
Please send money (check or money order) with your request.
It is acceptable to use the New York State Genealogy Application form to request a record, but not necessary. The town does not have an official form of its own.

The record sent to you is a photocopy of the original entry.

It usually takes 2 days to answer a written request for a record.
